Orthopaedic Surgical Associates & Merrimack Valley Spine Center (“OSA”) were recently notified of a data breach impacting a third-party vendor, ConnectOnCall. OSA takes the protection of patient information seriously. Following notification of this incident, OSA terminated the partnership with ConnectOnCall. Patients will receive notification letters of the incident directly from ConnectOnCall. Medical Record Requests

Orthopaedic Surgical Associates provides patients multiple ways to request copies of their medical records. There is currently no charge to obtain your medical records through our patient portal, Athena Health. Please note, if you request a copy of your images, there is a fee and the CD must be picked up in the office.
